Employee Contract

EMPLOYMENT CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF EMPLOYMENT is entered into by and between ACTIVE CHASE, INC., a corporation existing under the laws of the Republic of the Philippines with principal address at #7 RBNS Compound Poro Point, San Fernando City, La Union. represented by its President, Gidon Levy and hereinafter referred to as the EMPLOYER;

and , of legal age, Filipino citizen, single, a resident of , Philippines, herein referred to as EMPLOYEE;

WITNESSETH

The EMPLOYER hereby engages the services of the EMPLOYEE for the period from April 18, 2024 to , renewable for another 1 year upon satisfaction of the qualifications set forth in this contract, and by the willingness of the parties;

The EMPLOYEE shall render service to the EMPLOYER for the time schedule to be provided by the EMPLOYER. To monitor attendance, the EMPLOYEE shall be required to time-in and time

PAYOUT SCHEDULE: For services rendered from the 1st day to the 15th day of the month, the payout is scheduled on the 28th day of that month. For services rendered from the 16th to the end of the month, the payout is scheduled on the 13th day of the immediately succeeding month;

The EMPLOYEE shall well and faithfully discharge his/her duties, complying in good faith with all existing policies and regulations of the EMPLOYER, as well as with those that may be subsequently adopted after the signing of this contract and not inconsistent with any stipulations herein;

Should the EMPLOYEE desire to voluntarily terminate his/her employment, he/she shall serve a written notice to the EMPLOYER at least fifteen (15) days before the intended date of termination;

Even during the effectivity of this contract, the services of the EMPLOYEE maybe terminated for any of the just or authorized causes provided under this contract, the existing laws, or for any of the offenses carrying the sanction of dismissal enumerated under the Code of Discipline;

The EMPLOYEE’S salary earned during the last month immediately preceding the termination of this contract shall be released only upon submission of proof that the EMPLOYEE has been cleared of financial and property accountabilities to the EMPLOYER;

This contract shall be subject to the applicable provisions of the Code of Discipline and all existing and subsequent policies of the EMPLOYER, which are deemed incorporated into this contract;

During the effectivity of this contract, the EMPLOYEE shall not avail of any other remunerative employment other than employment that does not give rise to any conflict of interest or incompatibility with his/her work hours with the aforementioned establishment. To ensure compliance with this provision, an EMPLOYEE who wishes to avail of any other remunerative employment shall secure a written authority from the EMPLOYER. Any other employment obtained without the said written authority shall be deemed to be unauthorized and shall be a violation of this Contract and the applicable provision of the Code of Discipline;

The EMPLOYEE shall undertake not to divulge any personal, privileged and/or confidential information that may come to his/her knowledge in the course of his work and during meetings, and shall continue to do so even after his employment with the EMPLOYER.

The EMPLOYEE shall undertake not to divulge any information regarding their salary to anyone other than active chase management, or privileged and/or confidential information that may come to his/her knowledge in the course of his work and during meetings, and shall continue to do so even after his employment with the EMPLOYER. If employee breaches this agreement, they forfeit their holding of their salary upon immediate termination.

It is understood that upon execution of this contract, the EMPLOYEE has read and understood the rules and regulations of the Establishment as stipulated in the Code of Discipline.
